This form helps Octavyan choose students and families for research sessions on an early version of a guidance experience. Taking part is voluntary. Filling in the form does not mean you will be selected.

If you are selected, we send a consent form explaining the session in detail, and the session only goes ahead once you have agreed and the student is happy to take part. The student can stop at any time.
